This position works Monday through Friday: Evenings from 5:00 pm to 10:00 pm and overnight from 10:00pm to 8:00 am.
Will also work 2 holidays per year.
This position cover’s the following counties: Monroe, Livingston, Wyoming.
Candidates can expect to perform a start of care and 1-2 revisits per evening shift.
For the overnight shift, candidates can expect to be called out 1-2 times a week on average.
Please note this can vary: You may not be called out at night at all or you may be called out several times depending entirely on patient need.
Note
– Candidates must be able to orient Weekdays from 8:00 am to 4:30 pm for the first eight (8) weeks of employment.

RESPONSIBILITIES: * Develops care plans appropriate to the patients/families health care needs in conjunction Patient, family, physician, clinical operations team, and third party payers including obtaining medical orders and approval of medical services.* Utilizes resources, supplies and time efficiently in order to contain costs and increase the value of all services.* Completes the OASIS Assessment to accurately reflect the clinical, functional and psychosocial needs of the patient.* Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established time frames.* Refers to appropriate Health Care Systems, UR Medicine Home Care (URMHC) and community agencies/resources.* Communicates to the health care team members the outcome of the assessment/reassessment (if applicable) visit to ensure appropriate follow up occurs based on the needs of the patient/family s plan of care.* Maintains currently knowledge of health care reimbursement system and financial impact.* Displays an exemplary level of patience, courtesy, and flexibility.
Interacts with patients, physicians, and URMHC staff in a manner conducive to maintain positive relationships and to meet the goals and objectives of the agency.* Maintains current knowledge of all available URMHC and community based services.* Ensures problem resolution at the point of contact.* Maintains appropriate productivity requirements.* Adheres to Professional Home Visiting staff Performance Standards.* Performs other duties as assigned.* Responsible for the Key Performance Indicators (KPI s) established by their manager.* Assumes responsibility for his/her own continuing education, using formal and informal resources that contribute to professional self-growth.
QUALIFICATONS: * New York State Licensed RN with a minimum of one year of experience as a clinical nurse in a hospital or home care agency or BSN and a minimum of 6 months experience in hospital or homecare.* Working knowledge of health care reimbursement systems, government regulations and health industry accreditation standards relative to health care eligibility criteria, payer utilization, prior authorizations, and assessment/teaching standards.* Working knowledge of IVs and advanced wound care products.* Excellent organizational/interpersonal skills (including time management, patient visit prioritization).* Ability to effectively assess the physical and psychosocial needs of the patient/family and develop an appropriate plan of care.* Willing to utilize the technology as appropriate to meet the requirements of the job functions.
